By definition, IVF failure represents implantation failure.  Fertilized, growing embryos have been placed in the uterus, and failed to implant.  This disappointing result can be due to eggs, sperm, or the uterine environment.  While the science of implantation failure is far from perfect, over the years, a variety of tests and approaches have developed that can shed light on the problem, and help many patients conceive viable pregnancies after repeated failed attempts.

Over the past 20 years, I have evaluated many of the tests used to evaluate implantation failure, and through a combination of reviewing the medical literature and my own long experience, I have tried to select tests that either have good scientific evidence supporting them, have proven useful to me, or just make sense.  This website will describe an approach to implantation failure that is based on these three criteria.  In doing so, it will attempt to show which tests are well-supported by evidence, and which ones are more controversial.

For some patients, their own IVF experience and some of these tests will reveal that they may need an egg donor or surrogate.  While these are often not the immediately desired solutions, it is usually better to come to this decision earlier, and not after multiple IVF failures.

In addition to helping couples overcome infertility, IVF opens a window into why they may not be getting pregnant.  A systematic approach to implantation failure can help many couples conceive viable pregnancies after IVF has failed.  Sometimes, this evaluation will lead to the conclusion that an egg donor or surrogate is needed.  While not the most desired solutions for many couples, coming to this conclusion sooner than later can spare wasted dollars and considerable grief over repeated failed attempts.
